Research summary

  • Balance and proprioception training has one of the more mature randomized-trial evidence bases in exercise medicine, because falls in older adults are common, costly, and relatively easy to measure.
  • An influential meta-analysis of 54 randomized controlled trials found exercise programs reduce the rate of falls by roughly 16% overall, with meta-regression showing that programs incorporating a substantial balance-training component and a high total dose of exercise produced the largest effects.
  • More targeted meta-analyses of trials using balance-specific, multi-component, or perturbation-based training report comparable or larger reductions in fall risk, and individual randomized trials — from community-dwelling older adults doing treadmill perturbation training to frail nursing-home residents doing combined balance-and-resistance programs — consistently show fewer falls, better gait, and improved lower-limb strength versus usual care.
  • Because falls are a leading cause of injury, hospitalization, and loss of independence in older age, and hip fractures in particular carry markedly elevated one-year mortality, an intervention that reliably reduces fall rates has a meaningful, if indirect, effect on healthy longevity.
  • The main limitation is that most trials measure falls and functional proxies rather than all-cause mortality directly, so the evidence, while consistent and causally strong within its own outcome, has to be extrapolated one further step to connect it to lifespan itself.
