Research summary
- At least seven independent dose-response meta-analyses published between 2021 and 2024, including a synthesis of 15 international cohorts in The Lancet Public Health and an umbrella review, converge on a consistent pattern: higher daily step counts are associated with progressively lower all-cause and cardiovascular mortality risk, with the steepest risk reduction typically occurring between roughly 4,000 and 8,000 steps per day and more gradual additional benefit beyond that — meaning meaningful benefit occurs well below the popular 10,000-step target, which appears to have originated from a 1960s Japanese marketing campaign rather than research.
- This is one of the more robust bodies of evidence in this entire database, both because the exposure (steps, usually via accelerometer) is measured objectively rather than self-reported, and because the finding has been independently replicated by several different research groups using different cohort data.
- The main caveat is that, like nearly all physical activity research, these are observational cohort studies rather than randomized trials, so some residual confounding by overall health status cannot be fully excluded.
