Research summary

  • Contrast water therapy (CWT) — alternating hot and cold water immersion after exercise — has a substantial randomized-trial literature behind it, but it paints a genuinely mixed picture rather than a clear verdict.
  • The most consistent finding is for delayed-onset muscle soreness and muscle-damage markers: a meta-analysis pooling 18 trials found CWT produced significantly less soreness than passive recovery at every measured time point from under 6 hours to 96 hours post-exercise, and also reduced strength loss at each interval, and a 2024 network meta-analysis of 57 trials (1,220 participants) ranked CWT as the single most effective of four cold-based recovery methods for lowering creatine kinase, a blood marker of muscle damage.
  • Team-sport-specific evidence is more qualified: a meta-analysis of 23 studies in team-sport athletes found CWT improved perceived fatigue at 48 hours post-match, but unlike cold-water immersion alone, it did not produce measurable neuromuscular recovery benefits.
  • Directly countering the more favorable findings, a 2024 meta-analysis of 18 randomized trials found CWT produced no significant difference from a passive control group on either exercise performance or fatigue-recovery measures, and a separate 2023 meta-analysis concluded that cold-water immersion alone outperformed contrast water therapy on most recovery outcomes.
  • Taken together, contrast water therapy has real support for reducing subjective soreness and certain muscle-damage blood markers in the days after intense exercise, but the evidence that it meaningfully restores athletic performance faster than simpler alternatives — or than doing nothing — is inconsistent, and most underlying trials are small and carry a documented high risk of bias.
