Research summary
- Beyond its well-established efficacy for anxiety and depression, CBT has strong, replicated randomized-trial evidence for chronic insomnia — a flagship meta-analysis in Annals of Internal Medicine, later refined by a component network meta-analysis identifying which parts of the therapy drive benefit — with further meta-analyses confirming gains in insomnia patients with comorbid mental disorders or chronic disease, and improved quality of life.
- CBT also has meta-analytic support for chronic pain conditions including low back pain and migraine.
- A more recent and clinically important development is transdiagnostic CBT — protocols targeting mechanisms shared across disorders, such as repetitive negative thinking and rumination — which meta-analyses show is effective for emotional disorders broadly, including in internet-delivered formats.
- Across all of these applications the evidence base consists largely of randomized controlled trials rather than observational data, making this one of the more rigorously tested interventions in this category, though effect sizes vary by condition and are more modest for chronic pain than for insomnia or anxiety.
